Step 1: Visit the Duke MyChart Login Page: Open your web browser and go to the Duke MyChart (Which is now My Duke Health) login page. (https://my.dukemychart.org/).

Step 2: Find the “Forgot Username” Link: On the login page, look for the link that says “Forgot Username?” and click on it. However, if you do not remember your password, you may need to use the “Forgot password?” link first.

Step 3: Enter Required Information: You will be prompted to enter some personal information to verify your identity. Typically, this includes:

  • First and Last Name
  • Date of Birth
  • Email Address or Phone Number

Step 4: Submit the Request: After entering the required information, click on the “Submit” button.

Step 5: Check Your Email: If the information you provided matches the information on file, you will receive an email containing your username. Check your inbox (and spam/junk folder if necessary) for an email from Duke MyChart.
